Every student who leaves South Loop School must be given a transfer. If your child will be attending a school other than South Loop for the 2025-2026 school year, please complete a request for transfer form. To assist in preparing our records and transfer documents, we ask that you provide us with at least a 24-hour notice before the transfer is completed.
A request for transfer form must be completed for each child in the family who is currently enrolled at South Loop Elementary School.
*CPS Transfer and/or State Transfer form (where applicable) will be issued. Medical records and other records will be forwarded to the receiving school.
Please notify the school of your transfer by forwarding the email copy of your completed form (automatically sent to you upon completion) to:
