The Illinois Department of Public Health (IDPH) Immunization Section requests your immediate action to help avoid a measles outbreak in your school. We are contacting you because your school has an immunization protection rate for measles that falls below the Community Immunity Threshold (CIT) of 96%.
The United States is experiencing an unprecedented surge in measles cases across 22 states and jurisdictions. Measles is highly contagious and can be fatal, especially in school-aged children.
The Mobile Health Clinic will visit our Primary Building on Monday, April 11, 2025 from 10am – 1pm and Middle School on Wednesday, April 16, 2025 from 10 am – 1 pm.
The event is designed as an opportunity to access both school required immunizations and physical examination for state mandated health requirements

Medical Compliance
Immunizations and physicals are required for students entering ninth grade. It is recommended that students receive their immunizations and physicals prior to the end of their eighth grade year. View the Health Forms for Parents and Caregivers page for specific forms.
Physical Examination Requirements
All students must have a physical examination within one year of:
- entering schools in the State of Illinois for the first time at any grade level
- entering kindergarten or first grade, fifth grade, and ninth grade (ages 5, 11, 15 for ungraded programs)
- entering preschool up to age 6 (physical exam and lead screenings)
